Add more fields to widget

When new customer create ticket via widget on website it is a good idea to add there fields:

Phone no
Name

We use it very often now in our current support form on our website, we want to use widget from JitBit, but without these fields we don't like the idea to change our current form.

So, I can see 2 ways:
1. Just add those fields, so it will be visible when new users create ticket and if they fill it in this info can automatically go to the User Account.
2. Allow to add custom fields (but it is also should be a setting if we want to show it only to new customers/only in widget).
